Admission
Admissions Dental Hygiene
Admission into the Dental Hygiene Program is highly competitive, and class sizes are limited. To be accepted into the program, you must:
Complete a minimum of four hours in a dental office, observing a dental hygienist providing patient-care services. Observation verification forms are provided by the IUFW Dental Hygiene Program. They are also available on the website.
Submit the following information to the IUFW Dental Hygiene Program by February 1st of the year in which you are applying for enrollment:
IUFW Dental Hygiene Program application. NOTE: Copies of applications from other programs are NOT accepted.
IUFW Curriculum Information form.
IUFW Observation Verification form(s).
Official college transcripts from all colleges/universities attended.*
All applications must be received on or before the February 1st deadline.
Apply to the University and be admitted as a degree seeking student prior to May 25.
Prerequisite Courses
All prerequisite courses must be completed by June 1st with a grade of "C-" or better. No exceptions will be made for courses completed after June 1st, except BIOL 22000. Final transcripts must be received by the Dental Hygiene Program no later than May 25th.

*BIOL 22000 is a required prerequisite that must be successfully completed with a "C-" or better before entering the fall program. It will not be included in the prerequisite GPA for determining rank of admission into the program.
Minimum Credit Hours: 30
